Claims Procedure
To enable us to process your claim quickly and efficiently, please follow the instructions given carefully.
All insurers require information about you and your state of health when you make a claim. This information is recorded on our claim form. Since some doctors and healthcare providers prefer to enclose a claim form when they submit bills to insurers, we recommend that you take a claim form with you when you first visit a doctor, dentist or hospital.
