How to apply
To apply for a MediCare International insurance policy, there are two options.

Apply online
To do this, please visit our
Request information
section and follow the on-screen instructions. Here you can produce an online
quotation for your policy as part of the appplication process.

Groups up to 4
lives

Please use our normal
individual
application forms. Choose the level of protection and
geographical area you think is right for you

Fill out the application form, including partner and child details, where
appropriate

Calculate and enter the relevant plan premium according to the age of the
insured person and the level of cover. This must be the same for all
applicants. If you are paying in US Dollars or Euros please check our website
for details of current exchange rates or contact MediCare International by
phone on +44 (0) 207 816 2033.

Please remember, for applicants aged 65 and over, you must disclose all matters
which an underwriter would take into account when considering whether to
underwrite your insurance policy prior to cover starting. If such matters are
not disclosed, underwriters have the right to cancel the policy from its
commencement, which will lead to claims not being met.

You can choose to pay your premium quarterly for the addition of a 7.5%
administration fee – please ask us for details.

Cover will commence as soon as your application has been accepted and the
appropriate premium has been received.

CAPITALS/tick the relevant boxes.

Don’t forget to:
• provide details of previous claims experience if you have been running
another medical insurance scheme
• provide age/status details of all employees and their family members
• include details of your employees’ nationality and where they will be
located.

Look at the Benefit Schedule and select the type of plan you want for each
category of employee. The employees and their family members must be covered
under the same plan.

If you wish to pay the premium other than annually indicate your preferred
payment schedule (half yearly, quarterly) but note you must add 7.5% to your
premium payment for this option.

Sign and date the form and mail or fax to MediCare

Cover will commence as soon as your application has been accepted and the
appropriate premium has been received.

Bespoke
Schemes for Groups of 50 or more lives

We offer you the choice – build a group scheme from our standard benefits
choices or create a bespoke scheme that will match the needs of the employer
perfectly and benefit from our lower premium rates available on large groups.

Group Schemes: What happens next
Once MediCare International has received your Group Employer Quotation
Questionnaire, we will contact you with a quotation. If you decide to proceed
with the plan we will ask you to obtain completed and signed Employee
Application Forms (which are also downloadable from this web site) from all
employees to be covered and return them to us. Finally we will send you a
MediCare International pack for each employee with all their membership
information including the Certificate of Insurance and Identification Cards for
each insured person.
